Position Summary

Provide on-the-job support and training to adults with disabilities at community work site. Analyze job tasks and develop appropriate training techniques for those tasks. Provide written reports and information on each individual served. Supervise the daily operation of job sites consistent with the requirements of the program.

Responsibilities

  • Communication – Speak effectively; listen to others; write effectively; establish relationships; act with integrity; establish trust.
  • Maintain an accurate and up-to-date SET site manual.
  • Use Rise’s case-management software, myEvolv, to complete required assessments, treatment plans, attendance records, incident reports and contact notes.
  • Accurately records time and attendance for person served payroll and program billing. Completes recording of time and attendance in a timely manner.
  • Observe and record work performance in accordance with established norms for designated vocational programs. Maintain quality control and production records for the SET sites/customer as needed.
  • Develops and provides services based on individual goals identified by the expanded support team. Provides training and services in natural community environments.
  • Develop person-centered methods for training and supporting persons served in making progress on their desired outcomes. Provide the staff support identified in the person’s treatment plan.
  • Track person served progress in achieving the outcomes and objectives identified in their treatment plan.
  • Attends all team meetings as scheduled. Share updated information on individual needs, concerns, and employment site issues with the team.
  • Develop and write training objectives based on the expanded support team's response to goal areas.
  • Write annual reports, quarterly reports, treatment plans, support plans, individual abuse prevention plans, self-management assessments, case notations, progress reports, and incident reports, when needed, for one to six individuals.
  • Assist persons served in setting up and running all jobs assigned.
  • Carry out self-care and support services (toileting, feeding, personal hygiene needs) as assigned.
  • Communicate with other program staff and residential staff on a daily, or as needed basis, regarding program issues, caseload issues, staffing, and other related information.
  • Provide transportation services as designated by the driving schedule, drives and/or rides along the route. Provide transportation in personal vehicle as needed.
  • Assist persons served with boarding and exiting vehicles as needed.
  • Provide therapeutic programming as assigned.
  • Assist in general cleaning and caretaking of the building or work area as needed based on the SET site requirements.
  • Conducts oneself as a positive role model for persons served in terms of appearance, socialization, and teamwork.
  • Compliance with MVR (Motor Vehicle Record) and drug/alcohol testing program if required
  • Autonomy: Single-staffed SET site
  • Complexity of work performed at SET site
  • Degree of direct involvement in the customer work process.
  • Customer relations: Direct involvement with customer contacts
  • Startup of new SET site

Required Education, Experience, and other Qualifications

  • Technology skills: Comfortable working with computers, Microsoft Office software (Outlook, Word, Teams, etc.). and database management systems – Learn software systems to complete routine tasks.
  • Current, valid driver’s license and a driving record that demonstrates safe, legal driving habits
  • Compliance with DHS (Department of Human Services) Rule 11 and Rise MVR (Motor Vehicle Record) background checks required
  • Demonstrate good time management skills.
  • Build and maintain effective working relationships with SET site contacts and maintain business partner’s expectations for productivity, quality, etc.
  • Ability to run complex work sites independently.
  • High school diploma or GED

Preferred Education, Experience, and Other Qualifications

  • Previous experience practicing person-centered practices with people with disabilities or other barriers to employment.
  • Production/manufacturing experience.

 

Working Conditions and Environment

  • Hours of Work: Hours of work are typically daytime, Monday through Friday.
  • Travel: Must be able to travel to various locations throughout the day, in the Twin Cities area.
  • Work Environment: This job operates in a variety of environments including Rise’s office environment and production floor. The noise is moderately loud when on the production floor.
  • Physical Demands: The team member frequently is required to twist, stoop, bend, reach, pull, push, stand, sit and walk for up to eight hours per day. Able to lift and transfer people, with varying weights (50-75 pounds).
